Sarcome d’ewing - une localisation exceptionnelle: le sinus sphenoidal

Abstract
Ewing’s tumour is a primary bone tumour of high grade of malignancy belonging to the group of neuro-ectodermal tumours.
It can occur in all bones of the skeleton. However, localization in the facial bone is rare (1 to 2%) and exceptional in the
sphenoidal sinus. We report the case of a patient with Ewing’s sarcoma of the sphenoid with intraorbital and intracranial
extension. The symptomatology was headache and diplopia. The extension assessment was negative. The patient
received 4 courses of induction chemotherapy (VAC/IE). The tumour was judged not to be accessible to surgery, which is
why the patient received intensity-modulated radiation therapy (IMRT), at a dose of 56 Gy in 25 fractions for 5 weeks.
Then he received consolidation chemotherapy with 2 cycles of VAC/IE and 7 cycles of VAC. A thoracoabdominopelvic
CT scan (CT TAP) was performed 2 months after the 7th treatment, which noted the regression in size of the tumour
process centred on the orbital apex, currently, the patient is asymptomatic and a new CT scan was requested which
showed stable imaging. The ENT localization of Ewing’s sarcoma is very rare. Its prognosis has been considerably
improved thanks to a multidisciplinary approach.

Résumé:
La tumeur d’Ewing est une tumeur osseuse primitive de haut grade de malignité appartenant au groupe des
tumeurs neuroectodermiques. Elle peut survenir sur tous les os du squelette. Cependant, la localisation au niveau du
massif facial est rare (1 à 2%) et exceptionnelle au niveau du sinus sphénoïdal. Nous rapportons le cas d’un patient
présentant un sarcome d’Ewing du sphénoïde avec extension intra orbitaire et intracrânienne. La symptomatologie était
faite de céphalées et de diplopie. Le bilan d’extension distance était négatif. Le patient a reçu 4 cures de chimiothérapie
d’induction type VAC/IE. La tumeur a été jugée non accessible à la chirurgie, La raison pour laquelle le patient a bénéficié
d’une radiothérapie avec modulation d’intensité (IMRT), à la dose de 56 Gy en 25 fraction durant 5 semaines. Puis il a
reçu une chimiothérapie de consolidation type 2 cycles de VAC/IE puis 7 cycles de VAC. Un scanner CTAP d’évaluation
a été fait 2 mois après la 7éme cure, qui a noté la régression en taille du processus tumoral centré sur l’apex orbitaire,
actuellement le malade est asymptomatique et un nouveau scanner demandé a objectivé une stabilité de l’imagerie. La
localisation ORL d’un sarcome d’Ewing est très rare. Son pronostic a été considérablement amélioré grâce à une approche
multidisciplinaire.
