Rectal duplications are quite rare presentations. We present an unusual case of a congenital rectal duplication cyst in a neonate. The baby was noticed to have a bluish cystic discolouration of the gluteal area, which ruptured with meconium discharge. No other congenital anomaly was noted. Investigations revealed the presence of a rectal duplication cyst, which required surgery. The baby recovered fully with no complications.

Key words: Rectal duplication cyst; Congenital; Neonate.
