Intra-abdominal ectopic pregnancies (EPs) account for 1.4% of all EPs (1) with the majority occurring within the fallopian tube. EPs on the spleen are exceedingly rare, with only 26 cases published in English since 1974. We present a case of a splenic ectopic pregnancy (SEP) and perform a literature review to consider best diagnosis and treatment strategies.
