Efficacy of Erandmuladi Niruha Basti in Kala Basti Schedule for Hemiplegia: A case series

Background: Pakshaghata (Hemiplegia), a Mahāvāta Vyādhi (major Vata disorder) described in Ayurveda, is characterised by loss of voluntary motor activity and coordination on one side of the body. It closely resembles post-stroke hemiplegia in contemporary medicine. Vitiation of Vāta Doṣa (functional bio-regulatory principle of movement) is considered the principal factor responsible for neuromuscular impairment and functional disability. Objective: The present case series is aimed at clinically evaluating the therapeutic efficacy of Erandamuladi Niruha Basti administered in a Kāla Basti schedule in patients of Pakshaghata, using functional neurological parameters such as muscle power, spasticity, gait, and speech.Methods: This case series included five patients diagnosed with Pakshaghata and admitted to a teaching hospital. All patients received a uniform treatment protocol comprising Erandamuladi Niruha Basti administered as per Kāla Basti schedule. The therapy was combined with internal medications, supportive physiotherapeutic measures, and prescribed dietary regulations. Clinical assessment was conducted before and after treatment based on parameters such as spasticity, muscle strength, motor function, and gait. A composite percentage improvement was calculated based on changes in all assessed clinical parameters. Results: All patients showed noticeable clinical improvement following therapy. Reduction in spasticity, increased muscle strength, improved voluntary movements, and better mobility were observed. The extent of recovery varied according to age, chronicity, and severity of the disease. Conclusion: Erandamuladi Niruha Basti administered in a Kāla Basti schedule demonstrated beneficial outcomes in patients with Pakshaghata. The therapy appears promising; however, larger controlled clinical studies are required to substantiate these findings.
