To test whether inflammatory indices derived from first-trimester complete blood counts (CBCs) can anticipate placental abruption. We performed an observational matched case–control study (January 2023–February 2025), including 44 histopathology-confirmed placental abruption cases and 44 controls matched on maternal age and gestational week. From first-trimester CBCs we computed neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), systemic immune-inflammation index (SII), systemic inflammatory response index (SIRI) and monocyte-to-lymphocyte ratio (MLR). Group differences and diagnostic utility were assessed, including ROC analyses. Compared with controls, placental abruption cases showed higher neutrophil counts with elevated NLR, PLR, SII and SIRI, while lymphocyte counts were lower (all p
