The Safety and Efficacy Results of Tenecteplase in Patients with ST Segment Elevation Myocardial Infarction in a Center with No Possibility of Coronary Intervention

Abstract


Objective: It was aimed in the study to assess the efficacy and safety of tenecteplase in patients with ST Elevation Myocardial Infarction.

Material and Methods: Tenecteplase (TNK) was used in the management of STEMI patients who were admitted to our center with acute coronary syndrome and identified by electrocardiography, and recorded efficacy and safety parameters of the patients. TNK treatment was administrated as a single bolus dose  dependeng on body weight, through the brachial vein. The standard anti-ischemic and antiplatelet therapies were performed based on the current guidelines as additional therapy. Successful reperfusion criteria were determined according to clinical and electrocardiographic evaluations. Patients were referred to a tertiary center for the purpose of coronary angiography in the first 24 hours after applying TNK. Patients were reevaluated after a month from coronary angiography.

Results: Forty five patients (32 male, 13 female, mean ages 55.2±13.25) were included in this study. 32 patients had anterior myocardial infarction (MI), 11 had inferior MI and two patients had lateral MI. The mean time from the beginning of chest pain to the administration of TNK was 90 minutes. Clinically successful thrombolysis was reported in %84.4 of the patients. 1 patient had intracranial hemorrhage, 2 patients  died and 3 patients had myocardial reinfarction.

Conclusion: Based on these findings it should be considered that TNK may be a safe and effective treatment modality in patients with STEMI in a center with no possibility of coronary intervention.
