This article presents a series of 3 cases which have incidence of less than 1 percent and were encountered during the diagnosis and treatment of patients of hyperthyroidism. It includes thyroid acropachy a clinical feature in the chronology presenting after ophthalmopathy. Identification of the sign could prevent an unnecessary investigation into its cause. Carbimazole's rare side effect including renal damage and agranulocytosis, which are reversible with removal of the causative drug and hence timely insight into the side effect would help reverse the condition before irreversible damage ensues.

Key words: Agranulocytosis, Carbimazole, Hyperthyroidism acropachy, Rare side effects
