Objective: Breast cancer is the most common cancer of women. Currently ,favorable prognosis and cosmetic results are achieved through multidisciplinary approach. In situations where the patient refuses treatment or if the tumor is not treated appropriately breast tumor may reach large sizes or cause large ulcers and necrosis of the skin.
Case: Thirty one year old patient presented with a foreign body puncture of her right breast; the breast ultrasound revelaed an incidental 3x3 cm mass, which was later detected to be invasive ductal carcinoma. Modified radical mastectomy was performed in 2006 and reconstruction with Tram flap was performed in 2010. In order to obtain symmetry, reduction mammoplasty was performed in 2011 where pathological examination was negative for malignancy. Ten months later, the patient presented with a mass and 3-4 lymphadenopathies in the axilla. Without follow-up, the patient presented 2 years later with gross ulcerative painful lesion of 10x10 cm in size. Chemotherapy was initiated, however after 2 cures, the mass fistulized to the skin and mastectomy was performed, followed by split thickness skin grafting in another session.
