Multifocal Extra-nodal, Non-Hodgkin lymphoma (NHL) involving testis and larynx is a rare entity. We present a case of simultaneous involvement of epiglottis, vallecula of larynx and testis in a 56 year male. A PET/CT scan can initially find, and greatly characterize such an uncommon multifocal extranodal NHL presentation, and evaluate the extent of disease involvement, and later assess the efficacy of the treatment. Biopsy and Immunostaining of orchidectomy specimen and vallecular growth were suggesting of different subgroups of diffuse large B-cell lymphoma (DLBCL). There are no specific guidelines for deciding therapeutic modalities in these cases. We treated this patient as an advanced DLBCL with combined chemotherapy with immunotherapy.

Key words: Non-Hodgkin lymphoma, Diffuse large B-cell lymphoma, Multifocal Extranodal NHL, Primary testicular lymphoma, Primary Laryngeal lymphoma.
