Introduction: Diabetic neuropathy affects approximately 50 % of individuals with long-standing DM. In Ayurveda it may be correlated to upadrava of Prameha as mentioned by various Acharyas. The management of diabetic polyneuropathy by contemporary medicine presents with severe side effects. Ayurvedic management most of the times is free from untoward effects. A case is reported here who was benefited by simple Ayurvedic management. Case: A 65 years old male, known case of Diabetes mellitus-2 since 7-8 years complaining of burning sensation, pain, decreased sensation, numbness and pin-pricking sensation in both feet since 1 year was managed by Madhutailika basti, Dhanyamla dhara (local) & Oral administration of Atibala moola kashaya and Bhoomyamalaki choorna. Result: The patient got significant relief in subjective & objective findings (Vibration Perception Threshold) i.e. overall improvement of 8.75 volts in vibration sensation. Conclusion: Ayurvedic management by improving the clinical condition in diabetic neuropathy drastically decreases the chances of lower limb amputation thus improving quality of life.
