Safety and efficacy of Supreme Laryngeal Mask Airway versus ProSeal Laryngeal Mask Airway for ambulatory surgeries in Adult Patients.

Abstract

Abstract-
Background and Objectives: - The Supreme laryngeal mask airway (LMA) is a new, single-use, supraglottic device that combines the functionality of the ProSeal and Fastrach airways. Till now, there are conflicting results regarding the oropharyngeal leak pressure of the Supreme LMA versus the ProSeal LMA which is major determinant of effective ventilation and airway protection for LMAs. So, this study was designed to compare the safety and efficacy of the ProSeal LMA versus the Supreme LMA in ambulatory surgeries.
Methods- In this prospective, comparative trial, eighty adult patients undergoing elective surgery were randomly allocated to two groups: group P (PLMA) and group S (SLMA) of 40 patients each. Oropharyngeal leak pressure (OLP), insertion times, number of insertion attempts, ease of insertion and pharyngolaryngeal morbidity were assessed. Statistical analysis was performed using SPSS version 21.0 software using Student’s t-test and Chi-square test.
P < 0.05 was considered statistically significant.
Results- Oropharyngeal leak pressure in group P (26.65 ± 1.59 cm H2O) was comparable to group S (26.08 ± 1.67cm H2O) and both was providing adequate seal and effective ventilation. The mean effective airway time was significantly less in group S as compared to group P (14.80 ± 1.24 seconds and 17.80 ± 1.47 seconds) respectively. Mean number of successful insertion attempts, haemodynamic response and pharyngolaryngeal morbidity were comparable in both the groups.
Conclusion- Both SLMA and PLMA are equally efficacious and safe for maintaining airway and ventilation in paralyzed patients. However, Supreme laryngeal mask airway is easier to insert with shorter effective airway time than Proseal laryngeal mask airway.
