Papillary thyroid carcinoma with bone metastasis: Challenges in diagnosis and management

We report a 57-year-old female who presented with a progressively enlarging, painful left pelvic mass over four months. Neck examination revealed a palpable thyroid mass. Thyroid function tests revealed atypical central hypothyroidism. Histopathology examination confirmed papillary thyroid carcinoma (PTC) in the thyroid and pelvic lesions. Imaging revealed expansile lytic lesions in the left iliac wing, consistent with bone metastases. FNAB of the pelvic mass demonstrated a metastatic involvement consistent with follicular-variant PTC. She underwent total thyroidectomy with modified neck dissection, followed by levothyroxine therapy and plans for radioiodine ablation. Early recognition and a multidisciplinary approach are critical for optimizing therapeutic outcomes and improving patient quality of life.
