Vomiting can be encountered in many psychiatric diseases varying from depressive disorders to psychotic disorders. On the other hand, persistent vomiting is one of the important signs of various medical disorders. In this article, we reported a patient, who was diagnosed with obsessive compulsive disorder (OCD), who primarily complained of persistent vomiting. Though the patient was evaluated and treated by many different clinics, he could not get any positive response from different treatment methods for 7 years. We treated the patient with maprotiline (150 mglday) because of his immense anxiety and weight loss, and he got better dramatically in point of anxiety, obsessive compulsive symptoms, and particularly compulsive vomiting during a 20-day period. Although, maprotiline treatment is not used frequently in the treatment of OCD; the early dramatic recovery of the case is worthy to be discussed. Additionally, the importance of psychiatric consultation for patients with chronic vomiting is emphasized in this article.
