Abstract

Bilateral medial medullary syndrome is a rare type of stroke accounting for less than 1% of all brain stroke syndromes. It is a posterior circulation stroke resulting due to thrombotic or embolic occlusion of anterior spinal artery and small perforating branches from vertebral or proximal basilar artery, supplying the medial aspect of medulla oblongata. It has a characteristic “Heart appearance” in magnetic resonance imaging (MRI) of brain. We report a case of 63-year-old male who presented with acute onset of quadriparesis and altered sensorium. MRI of brain revealed heart shaped hyperintensity areas in bilateral medial medulla
